Steps to Begin Your Care Journey:

Referral Form Submission:

Submit the required referral form for the service you need.

Three Ways to Forward the Referral Form:

Confirmation and Intake Appointment:

• Our staff will confirm receipt of your Referral Form and contact you to schedule an initial intake appointment, typically conducted over the phone.

Intake Appointment Details:

• The intake session collects essential medical information and aims to set client-centered goals crucial for assessments and interventions.
  
  
• This session, usually held over the phone, may be conducted by any clinic staff.

Preparing for Your Appointment:

On the day of your appointment, please bring:

1. Current medication list (if applicable)
   
   
2. Any assessment reports related to your referral reason, such as psychology, school, medical, or professional reports.
   
   
3. Caregivers or companions are welcome to attend.
   
   
4. For children, a parent or caregiver must attend.

Arriving at the Clinic:

• Plan to arrive at Perley Health ahead of your appointment time.
  
  
• Present additional documentation, if applicable.
  
  
• A Clinician will meet with you to explain the evaluation process and address any questions or concerns you might have.

We ask that clients provide at least 24 hours of notice to cancel an appointment.
